The audience for the instrument is K-12 principals. It solicits information regarding school characteristics, the level of school involvement in the Local Systemic Change Program, the school's progress toward excellence in mathematics and science teaching, and principals' opinions about effective mathematics/science instruction. The questionnaire was developed and is administered to participating principals, as part of the core evaluation of the Local Systemic Change through Teacher Enhancement program.
